Developmental Disabilities Administration Services
Son Grace Inc, offers several programs through the Developmental Disabilities Administration (DDA) to support individuals in their chosen community settings. There are several waivers under which individuals may qualify and apply for services including Community pathways waiver, Community supports waiver, and Family supports waiver. Once approved for services, a Coordinator of community services (CCS) will be assigned and assist the individual and family with choosing the most appropriate service(s) to suit his/her needs.

DDA programs offer 3 categories of services: Residential services, Meaningful Day services, and Support services.
RESIDENTIAL SERVICES
Community Living (Group Home)
Son Grace provides housing and supports to individuals in apartments throughout Maryland. Individuals choose their homes and living arrangements and are supported to live a full life out in the community.
Supported Living
Individuals have the option to live in their own apartment/homes with supports provided by Son-Grace inc. Individuals may also live in agency owned homes and apartments with supports. Individuals may live in another person’s home and pay them to support him or her.
MEANINGFUL DAY SERVICES
Community Development
Individuals are supported to participate in community activities, create and maintain meaningful relationships out in the community.
Day Habilitation
Individuals attend a day program with their peers and participate in activities of interest including games, group activities, and individual therapy.
Employment Services & Career Exploration
Individuals are assisted with finding and maintaining employment in the community with staff supports and supervision as needed.
SUPPORT SERVICES
Son Grace Inc is an organized health care delivery system which means that we partner with external programs and services to provide a comprehensive care plan for individuals. In addition to residential and meaningful day services, support services include Personal supports, nursing services, transportation services, respite care services, family caregiver training and empowerment services and more. A combination of these services may be included to an individual’s plan as added supports to assist individuals in maintaining health, safety and independence.

Long Term Care Services & Supports
Community First Choice & Community Options Waiver
Provides supports to older adults and disabled persons to assist them in living in their own homes with supports as needed. Supports may include activities of daily living, transportation, nursing, home delivered meals, accessibility adaptations and more.

Private Duty
Individuals may also apply for direct care services through Son-Grace Inc. and pay out of pocket. A care plan will be developed tailored to the needs of the individual that includes one or more services including nursing, personal care, transportation. Payment will be made directly to the agency by the individual and or individual’s family on a monthly basis.
